What you’ll be doing

The Justice Health and Forensic Mental Health Network (the Network) Integrated Care Service (ICS) supports the existing systems to provide and integrated model of care for the patient journey from reception, through custody and into the community.

The Manager Aboriginal Health and Community Partnerships is an integral part of the ICS and is responsible for the leadership and management of the Aboriginal Chronic Care Program (ACCP) and Aboriginal Health Workers (AHW) to provide a culturally and clinically effective approach to Closing the Gap in preventable health conditions to achieve improved health outcomes and support community reintegration for Aboriginal and Torres Strait Islander people leaving custody.
This position will work collaboratively with the multidisciplinary team across Justice Health and Forensic Mental Health Network (the Network) to ensure service delivery, planning, monitoring and reporting of the Aboriginal Health Services and will build and maintain state – wide community engagement and partnerships in Aboriginal Health to provide an in-reach and outreach model of care and act as an ambassador and representative of the Network at internal and external forums and events.

  1. Applicants must be of Aboriginal descent through parentage, identification as being Aboriginal and accepted in the community as such. Aboriginality Is a genuine occupational qualification and is authorised under Section 14(d) of the Anti-discrimination Act 1997.
  2. A tertiary qualification in a relevant field, or a combination of study and relevant work experience.
  3. Proven experience in ability to analyse data, produce reports and assess and manage data quality within agreed timeframes.
  4. Proven experience in leadership, management and recruitment of a team and working autonomously to build networks to form collaborative working relationships and partnerships with internal and external stakeholders and agencies.
  5. Proven high level written, verbal communication and negotiation skills, including the preparation of reports, submissions and business cases.
  6. Demonstrated understanding of the health and social needs and the strategies/initiatives to improve the delivery of healthcare of Aboriginal and Torres Strait Islander people.
  7. Demonstrated experience in using Microsoft Applications, computer systems and data management, with a current unrestricted NSW Driver’s licence and willingness to travel across the state as required.
  8. Demonstrated understanding of the NSW Health Integrated Care Strategy.
