About the role
As a Home Care Planner you will be responsible for the planning and implementation of Community Models of Care for customers with complex clinical needs and supervision of staff to ensure customer focused quality care services. Supported by an experienced, professional team your duties will include:

  • Providing guidance, support and direction to staff in nursing care and service delivery on shift as required, as well as being available “On Call” as required by Bolton Clarke.
  • Leading case management for high level support programs i.e. Home Care Package Programs level 1 to 4; Department of Veterans Affair Community Nursing (DVA CN) and other funding programs such as CHSP
  • Upholding clinical governance and managing the quality management framework within Home Care services to ensure care standards are met
  • Coordinating the planning and implementation of a Model of Care
  • Providing direct and indirect clinical care to customers within scope of practice and ensure clinical stocks are maintained
  • Ensuring customer records, incidents and complaints are documented and maintained correctly and participate in any investigations as required
  • Providing support to clients between Woodford to the Bribie Peninsula and up to Samford (the successful applicant will be given a dedicated area)
  • Supervising and supporting staff and/or volunteers reporting to this position

To be successful, you’ll have:

  • Understanding of the Aged Care Sector as well as Home Care Package, and other programme guidelines and service requirements
  • Demonstrated ability to lead a team to provide positive customer outcomes
  • Bachelor of Nursing or other qualifications and/or training deemed equivalent
  • Current Nursing registration with APHRA as a Registered Nurse with a minimum of 4 years’ experience (essential)
  • Current Senior First Aid Certificate and CPR Skills & Experience
  • Working rights in Australia
  • Current Australian Drivers Licence
  • A pre-employment medical assessment may be required for this role.
