Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. Austin Health is world-renowned for its research and specialist work in cancer, liver transplantation, spinal cord injuries, neurology, endocrinology, mental health and rehabilitation.
An exciting opportunity currently exists for an enthusiastic and highly experienced Allied Health Grade 2 or Registered Nurses Grade 3B Care Managers / Intake Clinicians to join our Health Independence Program. The Health Independence Program encompasses many of the services that deliver health care to support the transition from hospital to home. These services can also prevent the need for a hospital presentation or stay, with people able to access these services directly from the community.
We have 2 fixed term part time positions available – 1 at 4 days week until 31 December 2021 and 1 at 3 days a week until 30 June 2022

To be successful in this role you must demonstrate/Key responsibilities will include: knowledge of hospital and community services and options for client support, great communication skills and the ability to provide a flexible approach to client care and support.
