We provide a range of services including in-home services for people with disability that aim to increase their independence, skills and quality of life. In addition, we provide residential housing and care for people with disabilities of all ages, plus young people with complex care needs related to behaviour, health and development.
About our Disability Program
Our Disability Program includes:
- Residential Care – we provide people with a safe and secure home environment, led by a team of dedicated, and caring staff
- In home care – our experienced staff assist in the home with domestic duties such as grocery shopping, meal preparation, household chores, assisted exercise and paying bills
- Community Access – we help clients access the community for educational, recreational and vocational purposes.
About the role
This is a permanent, full-time role reporting to the Operations Director – Disability Services. It is based in Tullamarine, Melbourne and will involve travel to client’s homes as required.
Role Requirements
As an integral part of a small team, the role involves the coordination of our residential and/or Community-Access clients’ support needs and associated staff. The role will involve being on-call for 2-3 days/nights per week.
This includes, but is not limited to:
- Plan, coordinate and implement agreed client care plans, service agreements and service delivery arrangements with clients and their carers.
- Develop activity/program schedules that meet clients interests and goals that include effective strategies for support.
- Conduct assessments of clients, liaising with appropriate carers, existing service providers, government agencies and other key stakeholders
- Manage staff rostering and any changes to client booking requirements
Selection Criteria
To be considered for this opportunity, you will have:
- Appropriate tertiary qualification in Disability (Diploma = minimum; Degree = preferred) or qualifications/experience deemed equivelent
- Significant relevant experience, including working with clients with autism.
- Excellent knowledge of NDIS
- Excellent knowledge of the Disability Quality and Safeguard framework
- Demonstrated experience in successfully performing the inherent requirements of the role.
- Demonstrated skill in rostering
- High level written and verbal communication: the ability to communicate clearly and concisely, varying communication style depending upon the audience and to build and maintain relationships with other service providers and community organisations.
- Attention to detail
- Australian Citizen or unlimited and ongoing working rights.
