The Role

Taking a person centred and best practice dementia care approach you will provide an assessment service for persons living with dementia that are showing a responsive behaviour. We are looking for your warm, engaging and collaborative approach in gaining further understanding of their dementia journey, psychosocial or other needs or triggers within their environment that might be contributing to the onset of a new responsive behaviour. 

This is a personally rewarding opportunity where you will support families and those involved in their care, advocating on behalf of the person living with dementia and improving on their quality of living through behavioural management recommendations/strategies and practical interventions.

The position additionally provides an opportunity to further educate and pass on your dementia knowledge to other health professionals in evidence based strategies so they have the expertise in how to successfully identify and manage signs of BPSD in people living with dementia.

The Candidate

  • Strong aged care experience working closely with persons living with dementia either in residential care, community, acute/sub-acute geriatrics or psychogeriatric care.  Prior experience as a Dementia CNC will be very highly regarded.
  • Senior clinical or allied health background eg. Nurse Lead, Clinical Care Manager, Occupational Therapist, Physiotherapist, or Senior Diversional Therapist / Senior Leisure and Lifestyle Co-ordinator within aged care where you believe you have strong dementia knowledge and experience.  Alternatively, you may be working as an ACFI Specialist, ACAT Assessor, Facility Manager, or as a Clinical Educator where you feel you would like to make a more meaningful difference at different stages of a person’s dementia journey and be pivotal in providing higher quality of daily living outcomes.
  • Prior experience in managing behavioural psychosocial symptoms of dementia and astute in identifying unmet needs or triggers within different care environments.
  • Warm and caring approach who can build rapport and trust with ease.
  • If you are an avid learner in dementia care or are currently studying or completed tertiary qualifications in Dementia Care, Gerontology or Health & Ageing we would to hear from you as it will be very highly regarded.
