About the role

This role provides complex care coordination in the rostering of services for clients using our Client Management System with the aim of assisting people to live independently in their homes. Care Coordinators work with Case Managers, clients, carers and external services in the provision of appropriate, targeted and quality services to our clients. This is a high paced and multi-faceted role requiring the ability to develop innovative approaches to meet the needs of clients.  

We are currently looking for an experienced Care Coordinator to join our Sydney Team at Beverley Park on a full time permanent contract.

What you bring  

  • Cert IV in Aged Care, Community Services or related field (or working towards a qualification)
  • Experience in Aged Care, Disability or Community Care
  • Previous experience in Care Coordination & Rostering
  • Knowledge and understanding of Person Centred Service Delivery
  • Current Drivers License
  • Current COVID19 vaccination
  • A desire to join a team committed to the care of others in a mission based organisation
